Prediction of pregnancy rate by blastocyst morphological score and age, based on 1,488 single frozen-thawed blastocyst transfer cycles Sakae Goto, M.D., Ph.D., Takashi Kadowaki, M.D., M.P.H., Ph.D., Satomi Tanaka, B.S., Hiromi Hashimoto, B.S., Shoji Kokeguchi, M.D., Ph.D., Masahide Shiotani, M.D., Ph.D. Fertility and Sterility Volume 95, Issue 3, Pages (March 2011) DOI: /j.fertnstert Copyright © 2011 American Society for Reproductive Medicine Terms and Conditions

Figure 1 Distributions of blastocyst quality by age groups. Blastocysts were significantly degraded in quality according to increased age (P<.001). The proportions of good blastocysts (BL-1 and BL-2: grade 4 and above) were 62.3%, 56.3%, and 41.1% in age groups of age 1 (22–33 years), age 2 (34–37 years), and age 3 (38–45 years). The prevalence of ICSI and standard insemination, the blastocyst quality, and age are presented. In all three age groups, the morphology of blastocysts derived from IVF were better than those from ICSI (P<.001, .031, and .036 for ages 1, 2, and 3, respectively). Figure 2 Odds ratios of (A) clinical pregnancy, (B) viable pregnancy, and (C) delivery as compared with the most desirable group (age, 22–33 and BL-1; grade 6, grade 5, and grade 4AA, 4AB, 4BA). No significant interaction was observed between age and blastocyst quality groups. Significant trends were observed for both age and blastocyst morphology groups (P<.001). Odds ratios of clinical pregnancy rate, viable pregnancy rate, and delivery rate were less than 0.5 in groups with age 3 (38–45 years), BL-5 (grade 2), and BL-6 (grade 1).
