Nuts and Bolts of IM4Q

History of IM4Q Planning Advisory Committee for ODP in 1997 developed Multi-Year Plan. The aim of the Plan was to determine how people being served and their families feel about the services they receive. The IM4Q is a data resource to the Administrative Entity and Providers for the Quality Improvement Plan. ODP is using IM4Q data to respond to CMS

Intent and Purpose of IM4Q
To Collect independent data from people who receive services from ODP and their family To ensure quality of life and satisfaction with services utilizing the Considerations Closing the Loop Process To provide data to stakeholders of the ODP system To ensure health of safety of people interviewed utilizing a major concerns policy

Who is involved in the IM4Q Process
Monitors Support Coordinators Person with Disability Team Members Family Members

Who is interviewed NCI (National Core Indicators) Total of 400 Across PA Must be receiving SC services plus one other service Ages 18 and older PFDS Must be receiving the Person Family Driven Services Waiver IM (Independent Monitoring) Ages 3 and older Can be receiving as little as SC Services only % of people living at home will be higher this year

Time line January- Second round of Regional Meetings August/ September
Sample lists are received by the AE. Training is conducted for monitoring teams March is the deadline to request additional samples is need April- Third round of regional meetings October- HCSIS usually opens to accept data entry First Round of Regional Meetings June all Data entry must be completed no later than the 30th Last Year Odessa did not open until December/ January July- annual Statewide training
