Evaluation Present a summary that includes a detailed and thoughtful evaluation of: your SMART goal your action plan actions that were successful and not so successful enablers that arose during the implementation process and how you made use of these, and/or how you made better use of them barriers that arose during the implementation process and how they hindered you achieving your goal and/or how you overcame them whether you achieved your goal partially, fully, or not at all, and why recommendations for future action: what would you do differently or what would you repeat and why what you learnt about yourself by taking this action that will help you in the future how your well-being has been enhanced by taking this action.

To do this, you will need to write the evaluation on refill and follow the steps below . . . . .

Evaluation . STEP 1: Write an introduction which tells me what you are going to be writing about. Introductions help to build the scene for the reader so they know what you will be talking about An introduction starter could be: In this evaluation I will be talking about . . . This is an evaluation which will look at . . .

Evaluation STEP 2: Explain how your Well-Being was being affected by your level of fitness before your training. This refers to your Mental & Emotional, Social, Spiritual and Physical wellbeing. Go back and have a look at your Mount Mission booklet. You have a page of reflection in there.

Evaluation STEP 3: Identify what SMART goal you set.
Explain what makes it a SMART goal. Explain what factors you used to decide on your SMART goal. (i.e. 400m pool test, cross country run, beep test)

Evaluation STEP 4: Explain what actions you took to improve your fitness over the past 3 weeks. State WHY you took each of these actions (what was its purpose) This includes: Clothing Type of training Diet

Evaluation STEP 5: Explain what enablers you had (things that helped you) and why they helped you during your training. Could include: Friends Music (IPOD)

Evaluation STEP 6: Explain the barriers you faced during your training (problems/issues). Explain how they affected you and what you did to overcome them. Things like: Weather Injuries Absent – funeral/sports trips/school trips??

EVALUATION STEP 7: State what your final Cross Country time was and whether you reached your goal or not. IF YOU DID – explain what steps you took along the way to track your progress. Also explain why you reached your goal. IF YOU DID NOT – explain what things held you back on the day and in your training.

Evaluation STEP 8: By doing this training, explain which areas of your Well-Being have been enhanced and HOW they have improved. Mental and Emotional: Physical: Spiritual: Social:

Evaluation STEP 9: What things would you change in the future if you were to do this again? Why would you change them? What future actions will you take regarding your fitness after this assessment? What things have changed in your life?

Evaluation STEP 10: Explain what you have learnt about yourself throughout this whole process. How will this help you in the future? How have your actions enhanced your overall Well-Being?
