1 Forms and Notes on Forms!
Resident Assistant Training

2 RA Weekly Report Weekly reports are due every Monday by 10am.
The RA Weekly Report is filled out online via the online programming page.

3 RA Duty Switch Form The RA duty switch form is used when an RA needs a duty switch. This form is used by some RA staffs. Please consult with your RD for the procedure they use.

4 RA Duty Log The RA duty log must be completed nightly by the Lead 1 or Lead 2 on duty. This is decided as a staff who completes this task. It is filled out as the night goes on to keep a detailed record of all that happens. The duty log is ed to your SRA and your building RD after your last round is completed. Note: The RD on duty does NOT receive a copy of this document.

5 Important Notes on: FAMIS
You should always keep track of the FAMIS work orders you place as you can search by the Work request number. It is also helpful to your RD the # as well. Any phone, cable or internet issue go through x4000. Please do not place through FAMIS. If a washer or dryer is broken that is not placed through famis, but through MacGray’s website Always be sure to enter separate work orders for each issue (example: broken window crank then another for a broken desk chair).

6 Important Notes on Weekly Walk Thru’s
Do not touch any illegal items (drugs or alcohol) regardless of your age. When in doubt, consult the RDOD. Place a confiscation notice on the item and in the student room when removing items. Keep your signs for the walk-thru’s up to date and visible!

7 Important Notes on RCRs
Write legibly, so that any one can read these forms. Students will appeal charges Keep it consistent when filling them out If you tell a student you are going to update their RCR do it in front of them so they know it is completed. Do not wait as you may forget! Always check to make sure work orders are completed if you tell a student you are going to place one for an issue! We don’t want to leave things hanging.
