4 Health Office Information- Mrs. Abbott
Health information required for kindergarten: Complete and current Immunization record Dental Screening Certificate (valid from 3 years of age) Blood Lead level (valid by age 2 and under age of 6 years)

5 Health Continued Health information recommended for kindergarten:
Complete physical Vision screen In addition, please complete the Annual Health Update (putty-colored form) and Health Screening Release forms The items listed above should be turned into the Health Office on or before the first day of school in August 2011 Please send a change of clothes

Registered for fall Will need to pay fees in August, as well as, turn in the rest of the forms Lunch accounts are set up using the registration forms Cost of lunch is $1.65 Cost of snack milk is $0.35 Cost of breakfast is $1.15 Transportation Request Form How to properly write a bus/town note

9 Kindergarten Readiness Skills
Fine Motor Skills- Holding pencil, scissors, coloring, zipping, snapping, tying shoe laces Gross Motor Skills- running, hopping, balancing Independence- able to separate from home without tears/clingy Reading Readiness- able to hold a book correctly, look through it independently, listen to a story with and without pictures Task Completion- able to finish a given task doing his/her best effort Taking Care of Own Personal Needs- coat, shoes, restroom Attention Span- able to sit and listen for at least 15 minutes or longer

10 Kindergarten Readiness Skills
Listening- without talking or interrupting for at least 5 minutes or longer, follow simple one step directions Body Basics- *Face person, *Make eye contact, *Use a serious/pleasant calm voice tone, *Keep a straight/relaxed body posture, *Have a serious /pleasant facial expression Line Basics- *Face front, *Hands to side, *Arms a length apart, *No talking, *Walk Share- classroom materials, teacher’s attention, and friends Self-Control- able to control my body and my emotions Letter and Number Recognition- able to identify some of each out of sequence

A . What should I bring to school on April 11th? Outside clothing necessary for the weather of the day Name tag – we have prepared one for your child to wear! If not eating the school lunch, a “cold” lunch

B . What activities will your child participate in during the day? Singing, Dancing Coloring, Cutting, Gluing Listening to stories Playing outside during recesses Eating lunch and enjoying an afternoon snack break Rest time Going to a “Special” class – Art, Music, or P.E. Playing in the classroom

C . Where will your child need to go at the end of the day? Please share your destination wishes with us – especially if they are different than they usually are. D . Where are my child’s projects and work samples that they will create that day? There will be envelopes sent to you later that will have your child’s work and some additional general kindergarten information in it. E . Conferences There is a sheet for those who are questioning their child’s readiness for kindergarten to sign their name who would like to have a conference with us on April 18th. We will call and arrange a conference time.
