Why an eDirectory? Because moms facing acute challenges (homelessness, mental health, domestic violence, medical challenges) can thrive with the right support! Because all new moms could use someone knowledgeable in their corner! Because there are great programs in LA to help both of these populations, but connecting with the right one can be complicated!

9 We need you! What the system will do: Your role:
Identify program(s) a family is likely eligible for Provide local program contact & referral info Your role: Gather the info needed in a warm manner “This is to help connect you to the right program.” Help the family decide which program(s) may be the best fit (if they qualify for multiple) Help the family enroll in a program Ideally, call the programs with the client to determine availability of openings, and, if needed, help the family fill out paperwork

Welcome Baby: Intended more for families who are not presenting high-risk needs; available for families delivering at one of the 14 participating hospitals, regardless of risk factors or socioeconomic status. Trained professionals provide personalized support and education during pregnancy and through the child’s first nine months of age. Programs for Families with Acute Challenges: Nurse Family Partnership: services provided by a nurse; evidence-based program intended for expecting moms enrolling prior to 28 weeks pregnancy, serving up to age 2; has sub-specialty for LAUSD teens MAMA’s Neighborhood: flexible program integrated with DHS medical services; particularly helpful for women with history of medical challenges and/or other high-risk factors (e.g., homelessness) Early Head Start: comprehensive evidence-based program that supports family goals and child development; has a focus on serving low income families & children with disabilities up to age 3; provides weekly visits and two monthly group socials to foster learning. Parents as Teachers: evidence-based support provided by a parent educator twice per month, as well as via group meetings; serves families through preschool-age; more child development/education focus Healthy Families America: evidence-based program intended for families who have experienced violence or other challenges (e.g. trauma, mental health issues, substance abuse); helps parents support their child’s development, school readiness and health; services may continue until the child’s 5th birthday Partnerships for Families: delivered by masters-level professionals; intended for families with a history of substance abuse, domestic violence or mental health Healthy Start: focuses on improving health, achieving healthy births, strengthening family resilience Families who prefer a remote program (Note-- in-person is recommended): USC Parents as Teachers Telehealth: Families participate in live, online meetings with parent educators.
