Consultation period: We’ve been listening
10th October – 30th November 2017 Speak with patients every day Listen to concerns Always look for ways to improve the care you and your family receive You tell us how much you appreciate nurses, carers, doctors and other professionals you come into contact with We’ve been listening We’ve been speaking with members of the public about Stockport Together for the last 3 years – have gained a lot of insight and feedback which has helped shape plans Need to make health and social care services which meet your needs

Who are we consulting? Everyone that wants to talk to us
People with a particular interest in health and social care The consultation is open to anyone – we’re asking anyone with information that needs to be considered to

Why now? We’ve been working with patients to trial news ways of working to: Improve access to GPs and other community medical services Improve care for our most vulnerable people Ensure our hospital is available for high quality care for those who really need it Why: ultimately to make Stockport a healthier place to live

What we’ve learned We can identify illness earlier and treat before people need to go to hospital We can help people recover quickly if we can get them back home quicker That people only want to be in hospital when it’s absolutely necessary and want to be out again as quickly as possible We can deliver in Stockport if our health and social care system works more closely together

How do we deliver what is required?
It’ll take time During recent listening events you said we should aim to maintain existing services until the news ways of working are up and running In July, we published plans focusing on 4 key areas Based on these plans – we received £19m of extra funding to ensure we keep our existing services while we get our new teams up and running

Supporting people to live healthier lives Improving access to GP practices and other integrated community health and social care services Increasing access to community mental health services Improving care for vulnerable people Reducing the pressures on hospital services especially those at Stepping Hill Hospital Why: ultimately to make Stockport a healthier place to live

Mental health Stockport, with Greater Manchester are committed to deliver the priorities set out in the Mental Health Five Year Forward View by 2020/21 Stockport locality spend on mental health is less than the England and Greater Manchester average Stockport’s overall performance on national mental health indicators show an average performance and the locality are committed to exceed performance to ensure good outcomes for the people of Stockport

There are high rates of children and young people and adults attending the emergency department because there are no alternative responses for people presenting in a mental health crisis The local mental health provider is experiencing financial challenges, which is having an impact on the capacity to deliver high quality services Out of area usage/non-contractual beds are commissioned because women presenting with severe and complex needs do not have access to a local female psychiatric intensive care unit

Mental health investment
Between 2016/17 and 2020/21 Stockport will invest and additional £9,672K in improving mental health services. Listed below are the key areas for investment:

Looking ahead In January 2018, the NHS and Council will decide whether to adopt this as their combined strategy Move the focus of health provision from care and treatment in hospital, to care and treatment provided at home or within the local community where possible First, we want to hear from you so we can take your views into account before we take any decisions We are recording your views today and you can also complete our surveys online
