IAG / DOS AzeusCare Phase 2 Electronic Care Monitoring PHB / CHC HIE, Data Governance & Health Technology Estates Plan Integrated Support Service Development Continued development of an integrated Information & Advice and Directory of Services with Health AzeusCare Phase 2 – CYPS Modules include Triage/MASH Development, Early Help, Sibling Recording, Portals (Child, Family, Provider, E-marketplace, Contract Management, Mobile working, SEND, Health Visitor & School Nursing, LADO, SGO Implementation of an electronic method of monitoring attendance of Care Agencies replacing the need for timesheets (including payment to providers) Developing the provision of Personal Health Budgets / Continuing Health Care payments to patients for the CCG Delivery of the East London Patient Record, CPIS, Data Warehouse, working towards providing a single view of information across all partners and services Maximisation of estates that exist across services (Use of Buildings) Realignment and the bringing together of support services across Strategic Commissioning – starting with Business Systems, Performance, Programme, Brokerage, Contracts & Commissioning
