Digital 2D versus Tomosynthesis Screening Mammography among Women Aged 65 and Older in the United States Screening mammography performance metrics are favorable in women aged 65 years and older and are further improved with digital breast tomosynthesis. Cancer detection rates are maintained and false-positive screening results are lower with digital breast tomosynthesis compared with digital mammography in older women, with lower abnormal interpretation rate (5.7% vs 5.8%; P < .001), higher positive predictive value 1 (PPV1) (14.5% vs 11.9%; P = .03), and higher specificity (95.1% vs 94.8%; P < .001). 73-year-old with a mass that is better characterized with tomosynthesis (b) compared with digital mammography (a).
