Figure 1 The diagnostic approach to a normal anion gap metabolic acidosis includes several urine and serum tests. The urine anion gap is one of the most important tests, but it merely assesses the robustness of urinary ammonium excretion in the setting of metabolic acidosis. A growing body of literature supports direct urine ammonium measurement in chronic kidney disease, and this measurement can readily be done by clinical laboratories.
