INCIDENT: One of HD driver in NCP crew wanted to move Mitsubishi canter from the well site after offloading drinking water. When he started the engine he did not notice that vehicle was in first gear. When engine was started vehicle jumped forward about 0.4m hitting IP, who was standing in front of the vehicle, and causing break of his right leg above knee and right hand above the fist. CAUSES: Failure to follow procedure Lack of due care and attention Failure to observe LEARNINGS: Apply defensive driving practice at all times Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction Always check that the vehicle is in neutral gear and clear of obstructions and people before starting.
