1 SWIFt: Future model Overall Aim
To support older people who are living with frailty with complex issues who are often socially isolated to improve their wellbeing and independence. Expected Outcomes Older people living with frailty will have improved health and wellbeing as a result of the service (Being active and healthy & Being able to do the things I enjoy) Older people living with frailty will feel less isolated as a result of the service (Being socially connected & Being able to do the things I enjoy) Older people living with frailty will live independently and safely in their own homes as a result of the service (Being independent) Aligned to what matters most to people living with frailty Target Audience The service will support older people who are living with frailty with complex issues who are often (though not always) socially isolated. All elements of the service will prioritise older people who are living without significant support and target older people (50+) who are: Experiencing moderate or severe frailty More likely to use health and care services Restricted to their own homes Living with dementia Carers of people living with frailty Experiencing mental health problems, such as anxiety and depression Helen

2 SWIFt: Future model April LCC assumed lead on commissioning from Leeds Older People’s Forum Continue to test and learn - with existing delivery partners (6 months funding secured). Funding secured but subject to governance process - expand into 6 additional Local Care Partnership areas with greatest need, defined by levels of moderate and severe frailty and deprivation: Seacroft Chapeltown Burmantofts & Richmond Hill Armley Beeston Garforth/Kippax/Rothwell  Sharing of learning Existing delivery partners providing support to share their learning to expand the service into these areas where they are in a neighbouring locality. Advice and guidance across the city in relation to best practice approaches for supporting older people from BME communities living with frailty. Broaden scope of citywide provision Support for older people living with frailty that have more complex physical and mental health needs Provide support and opportunities to share best practice amongst local SWIFt Workers.
