4. How to access Disclosures
To begin, click on “View your disclosures” from your Dashboard. If you do not see the “My Disclosure” widget, you will need to add it through your widget settings.

Once you have done that, you will see a similar screen as shown below. In this example, it shows prior submitted disclosures. If none have been submitted prior, this area circled below will be blank.

6. How to File a Disclosure
To begin, click “Add a new disclosure” as shown in the following example.

A window will then appear with various categories and their respective descriptions. Next, click on the appropriate disclosure category.

You will then be prompted to answer a series of questions. Please note, certain “Yes or No” and/or Multiple choice questions may surface additional questions, based on how they are answered.

9. Once you have completed the survey, click “Save” to submit.

To begin, click on “View disclosure details” from your list of submitted disclosure/s.

14. How to add a Message
To add a message to your disclosure, click on “Messages”. Next add your message in the text box, then click on “Submit”.
