As one of the over 190 communities involved in the campaign, we are not interested in who gets credit or who gets blame. We are interested in who gets housed. Together, we are a national movement to permanently house our most vulnerable neighbors. In the process, we are transforming the way Denton responds to homelessness and ensuring that a lifetime on the streets will be unheard of and unimaginable to our children. More than 50 volunteers surveyed people experiencing homelessness in an effort to identify those most vulnerable for dying on the streets. Volunteers did this in an effort to identify current needs and trends among those experiencing homelessness in Denton. Discuss Point in Time Count – Provide information about the difference in Surveys Using point in time count methodology, sites were selected based on visible homeless (known location) and also conducted at sites where homeless receive vital services like shelter, food and health care. 25 DENTON: THE NEXT 100 DAYS Goal: Get 5 Landlords to Donate Apartment for 12 months Obtained through the generosity of landlords. ENDING HOMELESSNESS IN DENTON

26 DENTON: THE NEXT 100 DAYS Goal: Select 5 people to house Goal: Build Case Management Team This client centered approach will identify each client’s unique needs and build a team of services to help them remain stably housed. The survey helped us identify the (5) most vulnerable homeless individuals in Denton and place them into their own apartment for twelve (12) months; along with enrolling them in case management and additional services to help them create a sustainable plan for their future housing and medical needs Client Center Approach / Collaborative Care – Using the survey data, the case management team will be assembled from multiple agencies and disciplines to address the specific needs of each client. Each cleints case management team will look different based on the needs of the individual. ENDING HOMELESSNESS IN DENTON

27 COST OF SUPPORTIVE HOUSING
Approx. $15,000 per year for permanent supportive housing (5 people X $15,000 = $75,000) For the next 100 days, we only need $1,000 per person in cash to move vulnerable into housing (5 people x $1,000 = $5,000) SOURCE: Corporation for Supportive Housing Statistics Discuss this as a project to let the agencies and community measure the effectiveness of a client centered/collaborative approach.
