The body-mass index is the weight in kilograms divided by the square of the height in meters. † Race was determined by the investigators. ‡ Hypertension was defined as a systolic blood pressure of more than 130 mm Hg or a diastolic blood pressure of more than 80 mm Hg. § A lipid disorder was defined by investigator reported diagnosis or as a low density lipoprotein cholesterol level of 100 mg per deciliter or more, a triglyceride level of 200 mg per deciliter or more, or a high-density lipoprotein cholesterol level of less than 40 mg per deciliter for men or less than 50 mg per deciliter for women.
