INCIDENT
A fully loaded articulated water tanker approached a sharp bend in the road at over
70Kmh (in an area sign posted as 40kph) and rolled over. The Driver and Helper
were trapped for some time until they could be rescued. The Driver sustained cuts to
his arm and head. The Helper sustained a fractured leg. The Driver was familiar with
the route and was aware of the sharp bend in the road.
ROOT CAUSES:
Over speeding
Lateral Learning
Adhere to the signposted speed limits
Slow down when approaching a bend
Recommended Actions:
Discuss this alert in toolbox talks and HSE meetings and
Distribute to all contractors
Post on HSE notice boards
Include in site HSE induction
