Full Water Tanker Rolled- Over Cause: - Mechanical failure (steering rod disconnected from the tie rod end & drag link) = NO STEERING ! - Driver lost control.

Injuries: Helper – Broken Leg and 3 broken ribs, cuts on forehead, Bleeding from the Ear and nose. Severity – 3P (LTI) Driver – Injuries to right leg Severity – 2P (MTC)

How many Roll-Overs did we have in 2002? (RTA –Rollover = 22) How many Roll-Over do we have in 2003 to date? (14 by mid-June) How could you, as driver, have prevented this Roll-Over? (Remember it was due to a Mechanical Failure)

HOW could this Rollover have been prevented?
Monthly Safety maintenance checks should include checks of the tie-rod ends. Qualified Mechanics must do the checks Maintain the vehicle to manufacturer’s standards Extra inspections if the vehicle works on rough roads. The driver had not attended DDC-04 for tankers.
