Psychology Learner Quick Start Guide: Practicum/Internship Application

Step One Definitions Initiating the process.
Before you can begin completing the application documents you need to initiate the process, by providing us with the site’s information. Click on Site Prospector on the left navigation bar. Select Submit New Site. Enter the quart.er you plan to begin practicum. Select the course name as outlined on your degree plan. Enter Preceptor’s name and address. Since you will only be submitting one application, you will enter 1 for site preference. Click Submit Prospect. You will receive an notifying you that your Preceptor was approved. This DOES NOT mean your practicum application, site, or site supervisor are approved. It does, however, provide official confirmation that you may: Begin to complete and submit application documents. Your Proposed supervisor will also receive an with a link to begin completing their portion of the application. Definitions Site Preceptor: Is used interchangeably with site supervisor. Rotation Details: Quarter in which you will begin practicum/internship Requirements: where you will go to work on required application documents. Definitions Step Two Completing & submitting required documents
It is your responsibility to ensure both your and your supervisor’s documents are received by the application deadline. Click Requirements to see a list of all of the application documents. You will see that some are required and other optional. Be sure to review all optional items to determine they are needed in your situation. To the far right of each document title you will see an Edit button, click on this to start each document. If the requirement is to complete a form, click on ‘Open New Form’ or select form from a dropdown list if provided. If the requirement is to upload a document such as a resume or proof of insurance, click on ‘Choose File’ and locate the file to upload. Once the requirement is complete, enter the date in the Completed On field and select ‘Complete’. If the requirement is still in process select ‘In-Process’. Steps Three & Four Definitions Submission of completed application
Ready for Practicum: this submitted once you have fully completed all other requirements & are ready to submit your application for review. Electronic Forms: here you will see a list of required forms and status of each form. Requirements: where you will go to work on required application documents. Rotation Details: Quarter in which you will begin practicum/internship Site Preceptor: used interchangeably w/ site supervisor Steps Three & Four Submission of completed application At the bottom of your Requirements page, you will see the Final Requirement. Once all of your requirements are complete, it is necessary to complete the Ready for Practicum requirement so that the Clinical Training Department knows that they can review your documents. Monitoring your application status Within the Requirements and Electronic Forms tabs you can see the status of each document, including if it has been approved by the Clinical Training Department. Once your application is reviewed, you will receive an from the Core ELMS address with comments from the Clinical Training Department. Here is where you will be informed if revisions are required. Step Five Definitions Monitor supervisor’s document submissions.
In order for your application to be considered fully completed and reviewed for the following quarter, all items must be received by the appropriate deadline. This includes the supervisor's required documents. You can see what items have been with these steps: Click on Scheduling on the left and then Rotation Schedule. Click on the site supervisor’s name under Preceptor/Site. In the section titled Additional Information for Students, the forms the supervisor has completed will be listed. Supervisors are required to submit the following items: Practicum/Internship Site Agreement Primary Site Supervisor Agreement Secondary Site Supervisor Agreement (optional) Primary Supervisor’s CV/Resume (attach/upload) Secondary Supervisor’s CV/Resume (attach/upload)(optional) Primary Supervisor’s License (attach/upload) Secondary Supervisor’s License (attach/upload)(optional) Definitions Electronic Forms: here you will see a list of required forms and status of each form. Ready for Practicum: this requirement is completed once you have fully completed all other requirements and are ready to submit your application for review. Step Six Definitions Review, Revise, Resubmit
Once you have completed the Ready for Practicum requirement, please allow the Clinical Training Department 3 weeks to complete their initial review. Monitor your for communications from this office regarding required revisions. Make revisions and resubmit for further review. You will follow the same process/steps as you did when you first completed and submitted documents. This process may have to be repeated more than one time before final approval can be granted. Save a copy of each completed form either by converting to a Word document or printing a copy for your records. Questions?
