Introductions Please give your name and role in this process.

Transition defined The process or period of changing from one state or condition to another. In the State of Wyoming the transition IEP is to be in place by the time the student turns 16 y.o. Discuss periods in life where the are transitions……some small some big. Examples starting kindergarten, starting high school, getting married, having kids. How do we prepare for these? What knowledge and skills are needed for these transitions?

Activity Please go to your respective board and list what you think your child/student/client should have to be successful in the adult world. Explain activity and make them feel comfortable and part of the process.

What does research show?
A recent study examined an online Transition Assessment and Goal Generator. Top predictors of post-secondary success were: 1) ability to effectively interact with others 2) ability to set and attain goals 3) employment during high school years This is research from the Zarrow Center for Learning Enrichment

Research continued 4) awareness of their disability

How do we get there? Team effort and it is our responsibility to our students to help them be as prepared as possible.

Indicator 13 34 CFR 300.43 Regulations state:
A coordinated set of activities that are results oriented, individualized, includes related services, community experiences, daily living skills and employment. What does the Federal Law under the IDEA require? This is condensed version. Does not apply to 504 students.

Process of Transition in school
Assessment Develop goals post H.S. Course of study to reach goals Monitor progress Annual IEP Refine/revise based on new information This a question bank that can be used for parents and providers.

Transition in Wyoming As of November 2017, Wyoming had 1944 IEP students age As part of our general supervision the State Department of Education reviewed 399 transition IEPs (about 20%). Most issues were was in a disconnect between the transition assessment and the post-secondary goals. Discuss this in terms of what the student is capable of, wants to do and what is realistic or pertinent.

In addition to that the State had 1433 504 students. (14-21)
Although 504 is not Federally Funded, this is significant in terms of our students in the State that have some type of disability. These 504 students will also need to know about their disability, accommodations etc. They need transition plans as well.

What are the common comments, concerns, wishes and hopes? We all need to be part of the team that helps our students transition to a successful adult life.
