U N I T Y The Unity Hub opening times: Saturdays 8am – 2pm Sundays 9am – 2pm Monday – Friday 6:30pm – 8pm To pre book please contact your own surgery or for on day appointments please ring: Who you might see at the hub. The reception staff will ask the nature of your problem to ensure you see the most appropriate clinician on the day. Practice nurse: Can help with a wide range of problems including wound dressing, chronic disease management, diagnosing and treating illnesses and ailments, providing advice about contraception, providing emergency first aid and treatment and giving advice and information about health conditions and ailments, stopping smoking and losing weight. Advanced nurse practitioner (ANP): These are trained nurses who have completed a specific degree course to extend their role to include examining, diagnosing and treating, including prescribing, for a wide range of common health conditions. Clinical Pharmacist: As a Clinical Pharmacist and independent Prescriber, they are be able to assist patients who need expert advice on their medicines, particularly in the treatment of long-term conditions such as diabetes or high blood pressure and offer specific advice on side effects. In addition they will be able to review your long-term medications. Clinical pharmacists also consult patients suffering from common minor illnesses such as colds, hay fever, diarrhoea and eye infections rather than having to consult a general practitioner. Doctor –A doctor will always be available if any of the above clinicians think you need further intervention, advice or if your condition is more serious.

Did you know your practice is part of the Unity group? This means you can access appointments at the weekends, week day evenings and Bank holidays. The Unity hub is based at: Pennfields Health Centre Upper Zoar Street Pennfields Wolverhampton WV3 0JH For on the day bookings when your surgery is closed. Telephone To pre book appointments at the Unity hub please speak to your own practice You may be offered an appointment with a practice nurse, clinical pharmacist, advanced nurse practitioner or a doctor. The receptionist will book you in with the most appropriate clinician to deal with your problem.
