3 CANADIAN ASSOCIATION OF AMBULATORY CARE CURRENT STATE STRATEGIC PLAN
To lead the nation and influence the practice of Ambulatory Patient Care through our research, educational agenda and practice innovations Vision: Mission: To transform the future of care provided in the ambulatory settings by offering a forum to network, share knowledge and skills and participate in leadership and research in the rapidly expanding ambulatory care environment. Definition: Ambulatory care is defined as any healthcare service or treatment provided in or out of a hospital setting that allows the patient to remain in the comfort of their own home 4

4 Shortening the statement to make it more direct and impactful
STRATEGIC PLANNING FOR 2019 – 2023 Review of the current strategic plan The CAAC Board met together on January 19, 2019 to review the current CAAC Vision, Mission and Goals to consider the effectiveness of the current Strategic Plan in the context of the future direction of the organization. Participants identified key themes in terms of opportunities for improvement of the VISION: Shortening the statement to make it more direct and impactful Include stronger language about CAAC’s role as the voice of Ambulatory Care nationally Promoting the CAAC as an organization that supports partnership with other healthcare organizations Include the patient in the statements 6

The VISION statement that was determined/suggested by the team is: Advancing excellence in Ambulatory Care across the nation. 7

6 Review of the current strategic plan: MISSION – THEMES
Participants identified key themes in terms of opportunities for improvement of the MISSION: Include a system approach Feature role in transformation of healthcare system (move away from Primary Care) Include diversity of membership, partnership across system Promote as a network rather than just a forum, more active role of leadership 6

The MISSION statement that was determined/suggested by the team is: Shaping the future delivery of ambulatory care through research, education, practice innovations and leadership in partnership with health system providers. Definitions: Future delivery – moving away from hospital-centric care to transitions of care across the continuum 7
