3. High potential incident – Unsecured working at height
Asset ONEgas: Operations West, Leman Echo
Date occurred: 14th December 2016
During night shift a person intervened whilst working unprotected next to an open area, walking across beams to do cleaning activities.
He could have felt down +/- 2 meter. Intervention was made by the night shift operations supervisor.
Although another team member also saw this dangerous activity he did no intervention. Nobody got injured.
