The reporting party can check on the status of their own report through the call center or website. The reporting party can speak with a live call center agent or use the website to submit a report. June 4, 2019

Three levels of anonymity:
Incident Report Form Locations and Departments can be selected from pick lists; or departments can be entered free-form. Three levels of anonymity: In addition to remaining anonymous or providing contact information, you may Remain Anonymous Toward Your Organization. At the organization’s request, MySafeWorkplace® can contact you for follow-up – which allows for ongoing communications while protecting your anonymity. The date and time of an incident can be specific or general (such as “last week” or “ongoing”). June 4, 2019

Importance of Anonymity Choices
You decide whether or not to provide any contact information! Stay anonymous and do not provide any contact information Provide contact information - or - You may choose to Remain Anonymous Toward Your Organization, but provide MySafeWorkplace® staff with contact information. If you choose this option, the Company will not know who you are and will not know any of your contact information This option is very helpful to enable communications between you and the Company. If you do not respond to messages posted by the Company via the MySafeWorkplace® website, the Company can request that MySafeWorkplace® contact you on their behalf. You then have the option to view the message and respond via the website; or provide your response to the staff at MySafeWorkplace® who will post your reply. June 4, 2019

Select Location and Department
Select the location where the incident occurred (a location selection is required) Select from the department drop-down list, enter a department name in the box provided, or bypass it entirely (department identification is not required) Select the date and time of the incident, either from the drop-down date and time fields, or enter an unspecified time in the box provided (such as “last week” or “ongoing” ) The only department associated with this location is ALL, and that is to be sensitive to your anonymity. Some departments may have only a couple of people associated with it and Corporate Express wanted you folks to have the ability to make the decision as to whether or not you would disclose which department/or area in this facility you work in. Report Distribution The individuals deemed appropriate to receive reports within the Company will log in and retrieve reports based on the incident type and location identified in the report If you chose “remain anonymous” or “remain anonymous toward your organization,” the Company will NOT know your identity from the report June 4, 2019

How to Check Report Status
You are encouraged to check status within hours of submission Two Ways to Check Report Status Go to and click “Report Status” Call and ask for “Report Status” You MUST have your access number and password - both of which are provided upon submission of your report Anonymously communicate with your organization by using the Message Board (“Talk to Your Organization” button on the report form) June 4, 2019

FAQ Is This Service Confidential?
Yes You determine whether or not to provide your contact information. You will have three options regarding anonymity when submitting a report: Remain completely anonymous Remain anonymous towards the Company, but provide your contact information to MySafeWorkplace® so the staff at MySafeWorkplace® may contact you, if and when appropriate Provide your contact information to the Company and the professionals at MySafeWorkplace® You may also name other individuals whom you would like to receive a copy of your report, understanding there is no guarantee. In these instances, the report will include that information and it is up to the discretion of the Company if they forward the report to those individuals. No one has access to your report except you and those designated by the Company to receive reports based on location and incident type. June 4, 2019

FAQ Can I report anonymously?
Yes MySafeWorkplace® was designed to allow for the completion of reports without the reporting party supplying a name or personal information. June 4, 2019

FAQ What will happen after I submit my report?
The report is transmitted in its entirety to pre-designated representatives within the Company All of the information is reported to the Company without editing or revision by the MySafeWorkplace® staff The Company can respond to the report as they deem appropriate June 4, 2019

FAQ Can I receive follow-up information after I have submitted my report?
Yes Go to and click on the “Go” button next to “Check Status”. Enter your password and the access code you received when you submitted your report. Be sure to click on “Talk to Your Organization” to see if there are messages from the Company to you. You may reply or post additional messages as well. Call the toll-free number ( ) and speak with one of the trained professionals at MySafeWorkplace®. Provide your password and the access code you received when you submitted your report. The staff can tell you the status of your report and any messages for you from the Company, as well as post any replies or additional messages you may have. Regardless of whether you use the internet or the call center, you must have both your access code and password to check on your report. June 4, 2019

FAQ What will the company do with the information?
All reports are accessed electronically by one of the Company’s pre-designated recipients. The Company has the ability to take whatever actions are deemed necessary to address the reported concern. This may include consulting with legal counsel or other external experts in order to determine the best course of action. While MySafeWorkplace® may make suggestions when asked, the Company is not obligated to accept or adhere to any particular advice. The Company has no duty to provide specific details surrounding their investigative efforts. June 4, 2019

FAQ Will MySafeWorkplace® refuse to accept any reports?
No MySafeWorkplace® will record and relay all information that is submitted through the web site or call center. Any and all information received will be transmitted to the Company, unmodified. June 4, 2019
