1 How to Report a New Incident to FSBIT

2 When a new incident occurs in your district….
Anonymous Link Does not require a password. Entry Only. Districts can have several anonymous links assigned. District Link Requires username and password. Recommended for only the risk manager and designees.

3 To submit a new incident:
Open Origami. Click on Incidents tab in upper right corner. *Depends on who is logged in. Will not appear in anonymous links.

4 To submit a new incident:
Click on ‘New Incident’ in the upper right corner.

5 To submit a new incident:
Select the appropriate claim type.

6 To submit a new incident:
The ‘New Incident Form’ will open and you can begin filling in the information about the incident. Any item marked with a red asterisk (*) is a required field and cannot be skipped. (The list of required fields and their descriptions are in Appendix A of the User Guide.) Any item with a down arrow ( ) in the text box has a drop down menu from which to choose the best fit. Since the first required fields are self-explanatory, start with ‘Occupation’ and refer to Appendix A.

Open the User Guide. Let’s go over the required fields on the form. Turn to Appendix A, page 8.

8 Use Appendix A to review all of the required fields with the participants. Spend most of your time on the ‘incident details’ section of the form (next slide).

9 Most of your time should be spent covering the fields under ‘Incident Details’.
Accident Location Area Type - general classification of the area where the accident occurred Accident Primary Location - A more specific classification of where the accident occurred from the Primary Location. Object Causing Injury - The specific item that caused the INJURY. Not to be confused with what caused the ACCIDENT.

11 To submit a new incident:
If you need to step away before completing the form, click on ‘Save Draft’.

12 To submit a new incident:
Once the form is complete, finalized and ready to submit to FSBIT, click on ‘Complete Incident’.

13 To submit a new incident:
The district’s authorized user will receive an stating that they have incidents to approve. The Incident Processor may be contacted to provide more information if it is needed. If the incident is submitted using the district link, no is generated. The incident goes directly to FSBIT. Once the authorized user reviews, edits and approves the incident, FSBIT will receive a notification from Origami.

14 To submit a new incident:
The incident will be reviewed by FSBIT staff to ensure completeness. It will remain ‘Notice Only’ or be made a claim. The adjuster will process the claim to completion. Remind participants that districts may be contacted for additional information if the form is not complete. This will delay the process.

15 Scenarios We are now going to use a few scenarios to practice completing the form. If you have access to Origami, complete the form with the participants while going through the scenarios. IT can delete the form you create later. Just don’t forget to tell them (via HelpDesk ticket)! If you don’t have Origami, use the images on slides 8-10 to walk them through the process.

16 Let’s file a new incident form!
Scenario #1 A bus driver stops to use the restroom at a grocery store and is involved in a slip and fall accident. Let’s file a new incident form! Have Origami open and ready to complete the incident form. There is a hyperlink to the Origami login screen in the last sentence on this slide.

18 Accident Location Area Type - general classification of the area where the accident occurred
Using our scenario of the bus driver who stopped to use the restroom, you could select “Food store” On-site accidents should also be as specific as possible as to where the injury occurred on site. Avoid using general descriptions such as “Campus”. Accident Primary Location - A more specific classification of where the accident occurred from the Primary Location. Some examples with our scenarios: If the bus driver who stopped at the store had made it to the bathroom when he/she slipped, you should code it as “bathroom”. If the driver fell outside of the store, you should code “parking lot” “Hallway/Corridor” would be appropriate if they were in-between the bathroom and parking lot.  When someone is injured on school grounds, there are many options to describe where on the grounds the injury occurred.  Object Causing Injury The specific item that caused the INJURY. Not to be confused with what caused the ACCIDENT The bus driver who fell in the grocery store, you would likely code as “floor/ground”.

Scenario #2: A food service worker is lifting a box onto the top shelf in the kitchen storage room. The box falls on her head and she collapses to the floor with a concussion. Okay. Let’s submit another form. Go back to Origami. Skip directly to the Incident Details.

Incident Occurs Entered into Origami by Incident Processor Approved by Authorized User at District Received by FSBIT staff and reviewed Claim Created Or stays ‘Notice Only’ Adjuster processes claim Claim closed
