NHS Health Checks The NHS Health Check programme is a systematic and integrated public health programme of vascular risk assessment and management which offers preventative checks to all those aged to assess their risk of vascular disease (heart disease, stroke, diabetes and kidney disease) followed by appropriate management and interventions.

NHS Health Checks …. to assess individual risk of coronary heart disease, stroke, diabetes and kidney disease, to communicate this risk in a way that is easy to understand, and to offer personalised advice, appropriate treatment and follow up to help individuals manage or reduce their risk- including being recalled every five years for reassessment.

Eligibility Criteria Clients are if eligible for a Free health check if: Between the age of years old Registered with a GP in Suffolk or resident in Suffolk Not had a health check within the last 5 years Must not be diagnosed (already on the disease register) or taking medication for the following conditions: Diabetes, Chronic Kidney Disease (CKD stages 3-5), Heart Failure, Stroke/TIA, CHD, Hypertension, Atrial Fibrillation, Peripheral Arterial Disease (PAD)

2. Risk Assessment Including diabetes and hypertension filters 1. Identification and invitation 3. Risk communication 4. Risk management including lifestyle advice and referral 5. Data collection management and sharing

Suffolk Pilot Commissioned Service.
Suffolk County Council working with GP surgeries that are not delivering NHS Health Checks currently or are underperforming to send an invite letter to their patients inviting them to have their health check done at any of the 30 participating pharmacies. Pharmacies can offer the Free NHS Health Checks service on opportunistic basis with no limit on the number of health checks that can be delivered on eligible patients PharmOutcomes used to deliver the Health Checks and the commissioner will have access to all data and be able to pay pharmacies through the system.

Suffolk Pilot – Payments
Pharmacies payment increased from to £30.00 per completed health check. Pharmacies pay for their own Cholesterol POCT machines and the mandatory EQA Scheme Enrolment at £ per annum for each pharmacy. Pharmacies cost and profit: Pharmacies pay for Cardiochek 25 TC/HDL test panels + pipettes (£100.01) Pharmacies to pay for HbA1c complete kit for 20 tests (£140 - not everyone is eligible for the test) Cost per 25 tests = £240.01 Pharmacies paid £30 /Check = 25*£30 = £750 (500 profit)
