Be sure to read through your summary and follow up with us as instructed

5. Many dermatologic conditions, like allergies, require life long management
Monthly calendars are very helpful to track your pet’s progress, and help us make decisions when changing any treatment protocols
If you have any questions, sending in your monthly calendars with a filled in update form is extremely helpful. It gives us a fuller picture of how things are going.

7. Each day is divided into three sections to allow for medications being given up to 3 times per day
Please fill in the key at the bottom so that we know which medications are being given
Along the top of each day is a scale from 1 to 10, with 1 being normal, and 10 being extreme
For many of our patients this is an itchiness scale, and for some it is a lesion scale. Please use whatever is appropriate for pet
