NQF 2152: Preventive Care and Screening: Unhealthy Alcohol Use Screening & Brief Counseling June 22, 2017

Purpose and Benefits 30% of the US population reports misusing alcohol/risky drinking Approximately 1/3 of these had a primary care appointment during the prior year Only 8.7% of people identified as “problem drinkers” reported that they had been asked and counseled about their alcohol use in the last 12 months. Studies show screening and brief counseling reduce alcohol consumption

Measure #431 (NQF 2152): Preventive Care and Screening: Unhealthy Alcohol Use: Screening & Brief Counseling- National Quality Strategy Domain: Community/ Population Health What Outcome is being measured? The quality of services provided for preventive screening for unhealthy alcohol use There is NO diagnosis associated with this measure Measure is reported by clinicians who perform the actions based on services provided and denominator coding

What needs to be collected? Tracked?
Patients aged 18 years and older seen twice for any visits or had at least one preventive care visit in last 12 months Screening Method (once per measurement year) Brief Counseling Intervention: One or more counseling sessions, minimum of 5-15 minutes that includes: Feedback on Alcohol use and harms Identification of high risk behaviors and situations for drinking Coping strategies Increased motivation Personal plan to quit drinking

Systemic Screening Methods:
One of the following Systemic Screening methods must be utilized (with thresholds for defining unhealthy alcohol use) AUDIT Screening Instrument (Score > or = 8) AUDIT-C Screening Instrument ( Score > or = 4 for men; score > or = 3 for women) Single question Screening: How many times in the past YEAR have you had 5 (for men) or 4 (for women and adults over 65 years of age) or more drinks in a day? (threshold response: > or = 2)

Denominator: Numerator: Check Age Check Service (Encounter) provided
If greater than or equal to 18 on date of service and does NOT report unhealthy use, do not include. If GT or = 18 on date of service and they DO report unhealthy use, include. Check Service (Encounter) provided If listed in encounter check for 2 eligible visits , if yes continue If No, check for a Preventive Care Visit If 1 PC visit, include in denominator. Denominator population all eligible patients. Numerator: Check pt. identified as an unhealthy alcohol user and received brief counseling: If yes: performance met If no: Check if patient not identified as an unhealthy alcohol user: If no: check documentation of medical reasons for not screening for unhealthy alcohol use If documented reasons: performance met If no: check if not screened or did not get brief counseling, for no reason: Performance not met Reporting not met
