Collaboration & Innovation for Community Health​ The Topeka Communities of Care was established in 2015 to address specific health needs identified in the Community Health Improvement Plan. While the focus of our work is related to Medicare and Medicare consumers, what we do helps all citizens in a community. Our job is to help communities identify the issue(s) and develop strategies to drive change. Following is an overview of the broad focus areas in Kansas Communities of Care: Reducing unnecessary hospital admissions. Reducing hospital readmissions. Reducing the number of Adverse Drug Events (ADEs). Improving community tenure.

Falling Less in KS Chronic Disease Self-Management Program (CDSMP) Diabetes Self-Management Program (DSME) Chronic Pain Self-Management Program (CPSMP) Medication List Safety Red Folder - medication profile forms - important documents that would be needed in the event of an emergency, or at any health visit Medication List Safety Poster Medication reconciliation training at ks.train.org Chronic Disease Risk Reduction Posters/Handouts for local health fairs
