Fig. 1 A vertical aortotomy is made along the greater curvature (a). After valve replacement, the margins of the aortotomy are everted and pledgeted mattress sutures are placed just beyond the proximal and distal ends of the incision (b). Using monofilament suture (4–0 polypropylene), a running horizontal mattress suture is placed as the first layer, reinforced by running simple suture of the same material incorporating a double thickness of aorta on each side of the incision (c).
