GRANULOCYTES ABO compatibility:
RBC’s may be grafted in the donor process, and these will react if not depleted or properly matched Must be ABO matched CMV status: Always important to assess in any immune suppressed patient Store at room temperature and give within 24hrs of collection: Avoids unnecessary complications, saves time (granulocytes have a short life span) Pre-med: Hydrocortisone may be at the Consultant’s discretion, but Promethazine and Paracetamol (unless LFT’s ++) are always indicated Medical and nursing supervision: The RN (or above) never leaves the patient unattended during the infusion and medical assistance must be less than 30secs away. Observations pre & post infusion, hourly during infusion or more frequent if clinically indicated Give over 1-2hrs: Minimises reactions No leucocytes filters: To use a filter is to remove the very cells you’re trying to give- granulocytes are leucocytes The matter of Amphotericin: This drug is know for considerable reactivity and depletes platelets and granulocytes.
