Health Status The standards all vary greatly. The one general consensus is that all health problems must be reported with the application and/or reappointment application. Some include illegal drug use in this section as must be reported.

Identity Joint Commission is the only accrediting body to mention this. There must be a mechanism to determine the applicant is the individual identified in the credentialing documents by viewing either a current picture hospital ID card or a valid picture ID issued by a State or Federal agency, such as a driver’s license or passport.

Board Certification Standards say that if the clinician is Board Certified it must be verified. Board Status cannot be used to solely determine if a clinician can be granted privileges.

CME Not addressed in NCQA, URAC or Medicare CoP’s
Otherwise, CME is required.
