With help from Collaborative staff we created a data base the showed us who our clients were, when & where they were being seen and what they needed to have done.

Now we could work within our team with other agencies to improve follow up and share care.
Creation of an HIV Primary Care flowsheet (IDC/Native Health)
Expanded role for clinic RN’s in shared care and case management
Regular communication and meetings with STOP Outreach team
