What the Specialist Apps Team do
Deal with all applications for entry onto the Specialist Register or GP Certificate of Completion of Training (CCT) Certificate of Eligibility for Specialist Registration (CESR) Certificate of Eligibility for General Practice Registration (CEGPR) Verifications and duplicates of certificates for GMC and previous bodies responsible for Specialist training / registration Coordinate GMC’s response to appeals against SR / GPR Applications for Out of Programme (OOP) approval Explain drs need SR to work as substantive consultant in non-FT NHS hospitals and GPR to work as GPs in any capacity (can’t otherwise work) CCT – approx. 6.5k per annum (5% time) CESR/GP – 90% time – 80% of these applicants non-UK PMQ 2

What’s a CESR? Eligibility in a CCT specialty
To be eligible to apply you must have completed: Six months continuous training in the speciality in which you are applying Or A Specialist Medical Qualification in the speciality in which you are applying Note that get small number of applications in non-CCT specialties – the eligibility criteria & standard of assessment differ 6

Eligibility in a non-CCT specialty
To be eligible to apply for a CESR in a non-CCT specialty you must have: Six months continuous training in any non-CCT specialty, undertaken outside the UK OR A Specialist Medical Qualification in any non-CCT specialty, obtained outside the UK. 7

How to apply and cost All applications can be made via GMC Online.
Once you have started your application you will need to submit it to us within 12 months. If we haven’t received it within 12 months we will contact you. Application fee: £1,600* *Fees due to change April 2019 – check website for details 8

Legislation governing the Specialist and GP Registers
Our main legal powers for maintaining the specialist register and the general practitioner register are contained in the Medical Act. In addition, there is a suite of subordinate legislation which describes how these powers are to be carried out. The Postgraduate Medical Education and Training Order 2010 The Applications for General Practice and Specialist Registration Regulations 2010. The CESR application process
Explain that diagram represents overall process once application submitted online. Takes around 6months based on time periods – can be longer if applicant doesn’t have evidence prepared, or shorter if they do. That’s why its really important to prepare before submitting. Will discuss each stage of the process in more details throughout presentation.

Stage 1 – Initial assessment
Eligibility check Structured reports Contact verifiers Hard copy evidence within 14 days Once submitted online, assigned to an adviser immediately throughout working hours, or first thing the next working day. The adviser will do an initial eligibility check (explain) based on the information given in the application We’ll request the structured reports for the nominated referees We’ll contact verifiers We will expect to receive any hard copy evidence within 14 days – if confirmed submitting hard copy evidence, we won’t start assessing electronic evidence until the hard copy evidence received

Structured reports We’ll ask you to nominate a minimum of four referees The royal college/faculty uses them to triangulate your primary evidence Choose people who can comment on your recent competence and breadth of your practice Two who work in your specialty Details on your personal attributes, skills and competencies First referee must be your current medical / clinical director 16

Stage 2 – GMC evidence review
Applications adviser reviews evidence: Check proformas match evidence Creates a checklist for you 60 days to gather additional evidence The verify portion of evidence once all additional evidence received The adviser will work through the hard copy and electronic evidence, which will contain about 1000 pages of documentary evidence: Check proformas provided where required for all evidence Create a checklist which will help identify any gaps in the evidence, anonymisation issues The checklist is sent to the applicant who then has a maximum of 60 days to gather any further evidence. We’ll then verify a proportion of your evidence directly with your verifiers – we need to be satisfied its an accurate reflection of your practice

Stage 2 – GMC evidence review
The adviser will work through the bundle, which will contain about 1000 pages of documentary evidence: Check proformas provided where required for all evidence – we’ll then contact all verifiers to confirm they completed the proforma Create a checklist which will help identify any gaps in the evidence, anonymisation issues The checklist is sent to the applicant who then has a maximum of 60 days to gather any further evidence. We’ll then verify a proportion of your evidence directly with your verifiers – we need to be satisfied its an accurate reflection of your practice

Stage 3 – Evaluation of your evidence
Royal college/faculty specialist assessment - 2 independent evaluators GMC QA of the evaluation GMC issues the decision based on RCF recommendation Once PSV sample verified successfully, the application is deemed closed and will be sent to the Royal College/Faculty for evaluation - they have expertise and are able to assess whether the evidence meets the required standards At this point we are subject to a 3 month legal deadline to issue a decision The RC will source two independent evaluators and have 7 weeks to return a completed evaluation In-house QA of the evaluation. We check that the it has been assessed against the correct standards, check that all evidence has been considered. We will revert back to RCF with any queries. GMC issues the decision based on RCF recommendation - appeal is against the GMC

Why are applications unsuccessful?
Haven’t demonstrated how well you have performed (WPBAs) Haven’t passed the standard test of knowledge within the curriculum or demonstrated equivalent Insufficient evidence of audits that have been undertaken, particularly on closing the audit loop Lack of evidence of research Specialising on one area Lack of management / leadership experience 22

Getting your decision If you’re successful:
Specialist registration is granted A copy of the evaluation is sent to you If you’re unsuccessful: A letter confirming you have not been successful, which outlines your options – Review or appeal A copy of the evaluation Recommendations which will help you in future applications 23

Review application You may choose to apply for a review of our decision if: You have additional relevant documentary evidence Or You consider there has been a procedural error You must apply within one year of the date on your decision letter. Review application fee: £695 24
