10 Tabletop-Form 1-Request for Assistance
(loaning cooperative) This letter confirms the express request made by this cooperative for your assistance in restoring our system and service to our members. In this regard we request that the loaning cooperative make available such of its employees as may consent to this work and can be spared to provide services to this cooperative. It is understood that this cooperative will retain the full right and responsibility of supervision and control of the work to be performed by all employees loaned by you; that you will pay the wages and expenses of such employees so employed and that this cooperative will reimburse you for the same. This cooperative will decide when it will no longer request the continued services of loaned employees. This cooperative maintains worker's compensation insurance for its own employees and adequate general liability insurance to cover the work and any injuries reasonably foreseeable resulting from errors or accidents which may occur in conjunction with the work. This cooperative will reimburse you for the use of your equipment, and understand that you are providing your own vehicle liability, property damage, collision and medical payment insurance. 11 Tabletop-Form 2-Agreement by Loaned Employee
Consent to Temporary Employment By                                                         (loaning cooperative) To assist                                                      (“borrowing cooperative”) during its emergency situation, I agree to become a loaned employee to provide my services to the borrowing cooperative, and understand that it shall have full right and authority to supervise and control the details of my work during the time I am loaned to it. I have volunteered for this service to assist the borrowing cooperative in restoring its system. As established by the Wisconsin Job Training and Safety Committee and summarized on page 8 of the WECA ROPE Manual, I will make every effort to insure there is no relaxation of safety during emergency restoration of service. I understand that in the event of my injury while engaged in this service that I will be covered by the worker’s compensation insurance carrier for my employer, the loaning cooperative, as described in the letter from Federated Insurance incorporated into the ROPE handbook. However, I understand that if I am injured in the course of this loaned employment, pursuant to the contract for insurance, I will be considered eligible for Workers’ Compensation benefits at my choice of the benefits for my home state or the state in which I am injured. I also understand that my employer will continue my regular salary, and that, my seniority and other fringe benefits will not be diminished because of my service to the borrowing cooperative.                                Dated  Employee

14 Food for Thought Most response times will be at least 2 hours unless responding help is for a short period and close by. Callbacks will come in from ½ to 2 hours. Some crews on standby depending on who responds in given timeframe.
