Seminole County H.O.P.E. Partnership between KAD Foundation and the Casselberry Senior Center Serving Hispanic Seniors 55+ throughout the County Community based program Timeline: May – December Target: 100 Seniors Budget: $20,000 The Seminole HOPE Program is a new program for Hispanic older adults in Seminole County, Florida. The program is modeled after the evidence-based Enhance Wellness and Enhance Fitness programs, endorsed by the National Council on Aging, with a 10-year history of producing outstanding health outcomes

Seminole HOPE Focus on Improving: Physical Activity Nutrition
Choices, Preparation, Portion Social Support Disease Prevention and Identification Funding will enable us to implement the HOPE program, expanding our efforts of serving senior citizens and the Hispanic population. Presently, a large number of Hispanic seniors frequent the Casselberry Senior Center, however, seldom participate in exercise and wellness programs. HOPE will actively recruit, appeal to, and reach sedentary, diverse Hispanic population. The objectives/anticipated outcomes of this project are as follows: To increase physical activity and access to physical activity. To improve knowledge of healthy diet and eating habits. Increase socialization among HOPE participants. To develop linkages with community referral sources for health and social support. To disseminate educational and project information.

Key Steps Screen Plan Action Questionnaires Needs Goals
Health Action Plan Tasks Action Participation in Activities Exercise, Education, Socialization The program happens in three steps: Screen, Plan, and Action. Screen: The participant completes a detailed health questionnaire comprising validated health measures. Examples of the health areas measured are confidence to exercise, confidence to manage health issues and work with physicians, physical activity, socialization, depression and anxiety, weight and nutrition, fall history and risk, smoking and alcohol use, and use of psychotropic and other medications. Together, the staff and participant review this report and explore the participant’s health goals. Plan: A Health Action Plan, developed by the participant with the help of the staff, focuses on areas of health risk that the participant chooses to work on. The staff is trained in motivational interviewing and behavior change and are ready to assist the participant to clarify goals and address motivational challenges. Specific tasks are defined, which will help the participant work toward meeting his or her health goals. Action: The participant moves into action with the support of the staff and/or contracted experts. HOPE offers physical activity, health education, and referral to support groups and additional services over an enrollment period of six months. At the end of six months the participant completes another questionnaire. We will compare the responses to the participant’s original questionnaire and provides immediate feedback to the participant on his or her success, not only in the areas he or she chooses to work on but also in all areas of health measured. Seniors—even the frailest—can improve their health and maintain their independence by incorporating physical activity into their daily lives. Seniors need access to enjoyable physical activity opportunities that are evidence-based and designed specifically to meet their physical needs – like the Seminole County HOPE Program.
