4. 2019 Initiative Overview
Improve access and availability of integrated, comprehensive services for older adults and their caregivers, delivered in a person-centered way. Two categories: Healthy Aging or Caregiving Cross-cutting goals: Workforce development or integration Grant amount: Up to $500,000 Time period: Up to two-year grants Applicants: Nonprofit and governmental agencies Due: July 11, 2019 Full RFP:

5. Healthy Aging Proposals Improve the health and wellbeing of older adults.
INNOVATION: Develop and pilot a new model to better support older adults that may involve nontraditional partners.
ACCESS: Improve access to current or expanded service array. This could be done through technology advancements, the integration of aging services into other health systems, increased workforce availability, or other strategies.
SYSTEMS: Address the structural barriers and inefficiencies that prevent various systems (e.g., education, health, social) from delivering care to older adults.
ISSUES: Address a significant issue area (e.g., social isolation) or various health
disparities impacting older adults.

6. Caregiver Proposals
INFORMATION AND ASSISTANCE: Improve caregivers’ access to resources. ASSESSMENTS: Provide caregivers with routine family-centered assessments to offer appropriate services reflecting specific needs. HOME-BASED OR COMMUNITY SERVICE DELIVERY: Assistance in early planning or respite coordination to avert crises. NAVIGATING HEALTH SYSTEMS: Support efforts to test a Caregiver Friendly Health System model. The holistic approach would strategically identify, coordinate with and support family caregivers post hospital discharge.

7. Planning Grant Opportunity
Overview: Planning grants are for pre-launch, piloting, planning, and design activities.
Grant amount: Up to $200,000
Time period: One year
Applicants: Nonprofit and governmental agencies
Requirements for a Planning Grant:
Community-based partners.
Address a specific topic or issue area.
Specific need to gather information.
Take between six months and a year to complete.
A formal, well-documented plan will result. 7

9. Evaluation Strategy for Your Project
1: Outputs and outcomes matter!
Outputs: “15 older adults attended our training session.”
“ The project served 15 caregivers.”
Outcome: “85% of seniors in attendance indicated that they learned something new that they will discuss with their physician.”
2: Tell us how you’ll know you were successful.
Evaluation is simply about how you know what you’re doing is working.

11. Poll
If the Health Fund offered potential Healthy Aging applicants a webinar to walk through the Evaluation Approach, would you be interested in attending?
NOTE: Polls are coded directly in Zoom. When crafting your slides, please enter the precise text you’d like to be displayed in Zoom and the answer options below. Then, enter that data into the Zoom Webinar portal to ensure consistency. If you’re asking a question about where the project will focus its work, the answer options should match the picture. Note: polls should not have more than 8 answer options, and Zoom will not allow more than 10 (after 8, the options get to be a bit tough to read on smaller computer screens, but you can show up to 10.)
Polls are pop-ups. Sometimes, participants with pop-up blockers installed on their web browsers will not be able to view the poll. Remind them of this verbally, and don’t panic!
Sample talking points: “We’d like to do a poll to see which area of the state your proposed project will focus its work on. Please select the appropriate number.” 11

Clearly identify if you are applying under Healthy Aging or Caregiving.
You must address one of our two cross-cutting goals.
Clearly identify the issue you will be addressing.
Think about your initiative in terms of long-term, sustainable impact.
Collaboration is key.
Health outcomes matter.

13. Timeline and Process Key Dates: Concept Papers:
Concept Papers Due (optional): May 30, 2019
Full Proposals Due: July 11, 2019
Awards Made: November 14, 2019
Concept Papers:
No more than two pages! Reviewed as they are received. Please address the following:
• Brief overview of the initiative, including proposed impact
• The cross-cutting goal you plan to address
• Key collaborative partners
