SEVERELY IMPAIRED NUTRITIONAL STATUS IN PATIENTS WITH LOCALLY ADVANCED PANCREATIC CANCER
PAN-NUTRIENT STUDY Drs. CM Lochtenberg–Potjes, JE Witvliet MSc, Dr. MAE De van der Schueren, H. Öztürk MSc 1
2
Introduction Pancreatic carcinoma is aggressive!
80% of patients already locally advanced of which 50% with evidence of metastases The carcinoma affects: Nutritional status Gastrointestinal functioning Caused by: Exocrine insufficiency Metabolic changes Objective: Provide quantitative data on nutritional and gastrointestinal aspects
3
Methods Patients Nutritional Assessment:
Included in PANFIRE study Nutritional Assessment: BMI and weight changes Energy intake (4-d food diary) Resting Energy Expenditure (REE) Hand grip strength Fat Free Mass Index (FFMI) Gastro-intestinal functioning: Energy intake (4-d food diary) and energy loss (3-d feces analysis)
4
BMI does not seem to reflect nutritional status sufficiently in LAPC Patients
5
Energy intake seems sufficient compared to equations, but equations underestimate actual energy needs by 30% in Lapc patients Figure 1: Median energy intake vs predicted (p) and measured (m) Total Energy Expenditure (TEE), kcal/day
6
individual energy absorption capacity ranges from not impaired to severely impaired in lapc patients
Moderately impaired Severely impaired No data LAPC patient number Figure 2: Absorption capacity (%) of individual patients vs not impaired, moderately and severely impaired
7
conclusions In patients with LAPC
BMI does not seem to reflect nutritional status sufficiently Nutritional intake seems sufficient if compared to energy requirement equations, but these equations underestimate actual, measured energy needs by 30% Individual energy absorption capacity ranges from severely impaired to not impaired at all
