1 Minimizing blood transfusions during abdominal aortic surgery: Recent advances in rapid autotrnsfusion  J.W. Hallett, M.D., Mark Popovsky, M.D., Duane Ilstrup, M.S.  Journal of Vascular Surgery  Volume 5, Issue 4, Pages (April 1987) DOI: / (87) Copyright © 1987 Society for Vascular Surgery and North American Chapter, International Society for Cardiovascular Surgery Terms and Conditions

2 Fig. 1 Mayo Rapid Autotransfusion Device (RAD). The RAD mixes aspirated blood with heparin, collects it in a plastic cardiotomy reservoir (1), washes red blood cells free of heparin, plasma, white blood cells, platelets, and other debris (2), and packages red blood cells in units of 225 ml with a hematocrit of 55% to 60% (3). The entire process takes 3 to 5 minutes, and the packaged blood is passed to the anesthesia team for reinfusion (4). 3 Fig. 2 Total perioperative blood utilization. Autotransfusion reduced homologous blood bank usage by 75%. 4 Fig. 3 Serial hemoglobin with rapid autotransfusion. With these hemoglobin levels, 68% of autotransfusion patients required no supplemental homologous bank blood during the entire perioperative period. 5 Fig. 4 Serial platelet counts with rapid autotransfusion. Although platelet count decreases with rapid autotransfusion, no significant difference was noted for platelet transfusions between autotransfusion group and control group.
