2. Physical Information Day: Saturday Date: April 14th
Time: 8:00 AM – 12:00 PM
Location: Northbrook High School
Pick up location: Your Coach should let you know where to meet and what time your school is scheduled for.
Cost: $10 or Free if on Free and Reduced Lunch
We will scan a copy of physical. We will keep the original on file. If you need a copy, please come see us the Monday after.

3. Physicals – what you need
You will need your physical form
You can get this form from your Coach, or
You can get it from
Just go to the Download and print link and click on Physical/Medical History link.
Glasses or Contacts if you wear them.
Dress in shorts and t-shirt.

4. Physical Form – Please fill out Correctly!!!
Write in Blue or Black Ink! – This is an Official Document. All Official Documents need to be done in blue and black ink. Not just here, but for everything in life. Pencil can be erased!
Name – Preferably Name spelled like it is in Rank One.
Grade – Please put grade you will be in for the school year!
School – Please Put the school you will be attending for the school year!!

5. Physical Form – Please fill out Correctly!!!
History – Please have your Parents help you with this.
If you answer yes to any question, you must explain because it can alert doctor to not pass you on physical.
If not enough room to explain, use a separate piece of paper.

6. Physical Form – Please fill out Correctly!!!
Student Signature – means you sign
Parent signature – means your parent signs.
For School Use Only – means for us to use. Please do not sign here.

7. Physical Form – Please fill out Correctly!!!
Only fill out the top line on the back of the physical.

8. Physical Form – Doctor’s area
If you go out on your own to get a physical, Doctor must check one of the options.
The doctors information must be completely filled out with a Doctor’s Stamp.
If NO Stamp, the Athletic Trainer or Coach will have to call the doctors office and verify that the physical is done. Physical will not be valid until this is done.
