2 Fig 1 The patient is intubated with a narrow lumen endotracheal tube. This tube is connected to an uncuffed tube of the same caliber. The tip of the endotracheal tube is pulled out through the tracheostomy and is inserted into the proximal part of the vertical limb of the T-tube. The cuff is inflated so that both the T-tube and the endotracheal tube can be pulled together.
