Osmotic nephrosis is characterized by marked swelling of proximal tubular epithelia that contain abundant clear cytoplasmic vacuoles. Compared to the isometric tubular vacuolization seen with calcineurin inhibitor toxicity, the cells appear more swollen and the vacuoles are larger, more abundant, and more diffusely distributed.
