2 Fig 1 Geometric basis for ring design. (A) Three-dimensional coordinates of aortic root structures were digitized from computed tomography angiograms in 10 patients with normal valves. (B, C) Average valve geometry was determined by ellipsoidal three-dimensional least squares regression analysis. (D) Average three-dimensional coordinates of the leaflet-aortic junction, in red, modeled normal annular geometry, and (E), three-dimensional coordinates were used to mill a one-piece titanium ring covered with Dacron. Green ellipses in B and D illustrate the normal 2:3 elliptical valve base. (F) An endothelialized ring is shown from the ventricular aspect in a calf study after 2 months of implantation [11]. (L = left coronary cusp; N = noncoronary cusp; R = right coronary cusp.) 3 Fig 2 Operative views of the first 2 patients' valves before and after repair. The markedly dilated and pathologic annuli are evident before repair. Green arrow is fracture of right coronary leaflet producing prolapse. Blue arrow is fracture of left coronary leaflet with prolapse. See text for other details. 4 Fig 3 Two orthogonal transesophageal echocardiography views for the first 2 patients before and after repair. Both patients had severe aortic insufficiency preoperatively that was reduced to mild or none after valve repair.
