Bio Dan Krohn is a Community Partnership Specialist at TMG. For the last 12 years, his work has been focused on partnering with people to build full lives in their communities by using his expertise in Community Based Supports and Person Centered Thinking. Dan has worked with TMG since 2017, using his talents to support people, families, schools, and service providers in the successful navigation of the Wisconsin IRIS Waiver. He lives in Milwaukee with his wife and two boys.

Workshop Objectives LEARN ABOUT a framework for person-centered thinking and self-direction that empowers individuals with disabilities to enjoy full citizenship and maintain choice and control over their long-term services and supports. LEARN ABOUT individuals whose life experience includes control and choices leading to full citizenship. LEARN STEPS and strategies to supporting opportunities for advocacy and empowerment. Speaker notes: Understand the values and practices that form the foundation of a culture of partnership in. Understand how person-centered thinking can be effectively operationalized and integrated into the fabric of a self-directed LTSS program. Reflect on your relationships through the lens of partnership and person-centered thinking. Apply the partnership lens to the organization or program you work with.

Who is TMG? Leader in community-based long term supports, quality management and service design for 30+ years. IRIS Consultant Agency (ICA) since 2008, partnering with over 15,000 people. Self-Directed Personal Care (SDPC) Oversight Agency supporting over 8,000 individuals who have chosen SDPC. Quality oversight for community-based services since 1986. Health and human services system analysis, design and training. Locally-based operations with over 600 staff in communities throughout Wisconsin. Speaker Notes: TMG began its rich history in 1986 providing oversight to the newly developed community options programs supporting people to live in the community. We were the first company to be awarded the contract to provide this quality oversight and have worked closely with the Department of Health Services to develop the processes and systems. In addition to this quality oversight, TMG provided consulting services. These consulting services were primarily in the area of long-term care and quality. Slide Bullet: Locally-based operations with 600 staff in communities throughout Wisconsin. TMG remained a relatively small company of approximately employees performing this quality oversight until the growth of IRIS and Family Care throughout Wisconsin. In 2008, we became the first IRIS Consulting Agency and now partnering with over 13,000 people. In 2009, TMG began work as the Oversight Agency for Self-Directed Personal Care. SDPC connects to over 5500 individuals using IRIS. We have almost 600 locally based staff throughout Wisconsin. In February 2016, TMG was acquired by Magellan Health. This was part of a long journey who wanted to retire, and more importantly to find a partner who shared TMG’s mission and values and had the same commitment to continue to cultivate the excellence in self-determination and self-direction and was already happening here at TMG and then look at how that could be exported to other states. Clearing the Path Ahead
TMG partners with people to: Develop valued roles and community connections. Identify their short term and long term goals. Access local resources. Create individual support and services plans. Support responsible use of their plan budgets. Support people to find meaningful days through employment and community engagement. Start by talking about the relationship between DHS and TMG, and then talk about the evolution of the IRIS program. Then talk about the reason we do this: to support the people we partner with. This is slide 5 from the SD Conference presentation, with Stacy's picture on it. It talks TMG’s role in partnering with people who self-direct. Speaker’s Notes: TMG partners with people to assist them in being successful when using a fully self-directed program. We take steps to make certain that people feel supported in a way and at a level that is meaningful to them. The type of support can look different for each person, but one aspect is always consistent-the person is always in “the drivers’ seat”. TMG is in the passenger seat as their partner, helping navigate or spotting the resources along the way.
