Create binders for originals, staff education/meetings Label everything  Post Mission and Vision Statements Daily Check-ins with staff Create Lists Shopping List Craft Schedules

Does your progress note match the care plan? Does participation sheets reflect the information in progress notes? Does participation sheets match the care plan? Does your care plan match the assessment? Are your care plan interventions actually occurring? If you document behaviors during activities are they care planned? Does your room visit list match room visits being care planned?

11 Work Smarter, Not Harder
Self Audit Create a binder for interdepartmental audits to use for tracking/trending Charts - During the MDS/Care plan review process Department – Once monthly If a trend is identified, add to quality assurance process immediately If a trend is identified then audit “like residents” If the trend is identified as wide spread problem then audit ALL residents Total census audit is time consuming, but WILL potentially avoid a deficiency.
