5. Consumer Survey Update
203 surveys collected from 7 collection sites
Majority of respondents were Medicaid recipients
Survey respondents identified the following as barriers:
Emotional/Mental Health
Access to Care
Affordability
Accessing providers
Transportation
Navigating the system
Access to food

6. Social Determinant of Health Priority Selection
Social Determinants of Health = conditions in the environments in which people live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.

7. Updated LHIN Contract
Membership limited to direct healthcare providers as 50% of membership
Must identify and frame a list of SDoH priorities relevant to region
Meetings will be open to the public and advertised through social media, newspaper, etc.
Must identify and review resources in community that strengthen the local health care delivery system (housing, transportation, nutrition programs, support groups, resiliency building programs, etc.)
Assist GCACH in identifying priority healthcare workforce needs in our community
Share with GCACH info about local events, programs, and training materials relevant to goals (Ex: Programs addressing opioid crisis)
