Ocular History 41, male, British
3 week history of acute VA decrease OD, no pain no redness, no h/o injury or drug intake H/o rheumatoid arthritis on oral Prednisolone 10mg/day and oral Methotrexate no previous ocular history H/o bitten by a cat few weeks ago

First Presentation VA: OD 6/24, OS: 6/6 Colour vision: 15/15
AC no cells, mild vitreous cells

First Presentation – Fundus and FLA OD
exudates and edematous disc with peripapillary hemorrhage, confirmation of findings on infrared picture and leaky disc on FLA

First Presentation - Investigations
Serology positive Bartonella titer > 2500

Diagnosis Neuroretinitis, suggestive for Bartonella induced based on
the positive serology and the clinical findings

Treatment Doxycycline 100mg twice daily for 2 weeks
Given for six weeks

Follow up – After 1 Month OD
less exudates and less hyperemic disc
9
Final Follow up – After 4 months
lesion completely resolved at end of six weeks vision resolved completely no further relapses
