Contractor was hired to remove two oil breakers from transmission substation. Note the hose in substation draining oil from oil breaker prior to moving breaker. These breakers were being replaced with gas breakers.

Warning tape Notice the Danger/Warning tape around the breakers. Crew that responded to the outage noticed that tape appeared, depending on how you looked at it, to enclose the breakers being replaced. At first glance this could look to be the area cordoned off to stay away from.

Metal tape measure used to measure bushing height
Contractor was hired to pump oil and haul breakers. Contractor who was already pumping oil walked up to energized breaker and with a metal tape attempted to measure the height of breaker.

Contact point on bushing
Contractor pumping oil and hauling breakers attempted to measure height of oil breaker. Unfortunately it was an energized breaker, not one of the two that were open, tagged and grounded. Tape made contact hear and we believe arced to the side of the tank.
