Figure 2 Simeprevir-sofosbuvir–related diffuse alveolar damage. A, Diffuse alveolar damage is evident in the surgical lung biopsy specimen, with hyaline membranes (hm) readily visible. The process was not uniform, with spared areas visible (hematoxylin-eosin, original magnification ×100). B, At higher magnification, intensely vacuolated type 2 cells and macrophages (v) are present with scattered intra-alveolar eosinophils (arrow). These combined features are highly suggestive of medication toxicity (hematoxylin-eosin, original magnification ×400).
