In the Netherlands, this process is related to autonomy and it can be done by simple registration on a specific website. The role of relatives: e.g. 18 years old guy, give his consent for donation before his death, his family refuses that. So here, we as a doctors should talk to family and try to change their mind (in ethical point of view). In Spain, Australia and other countries If you did not say that I do not want to donate, they will take your organs after death. Reciprocity: the practice of exchanging things with others for mutual benefit, especially privileges granted by one country or organization to another. So If you are registered as a donor, so you are eligible receive an organ. But if someone have HIV HCV so he can not be donor (non ethical) There is a consent for donation should be applied by family in Saudi arabia.
