4 New Employee Documentation
This presentation is designed to be an overview of the importation documentation required for your employment, the policies and procedures set forth in the County handbook and other important information regarding your benefits. Please visit the County’s website under employee tools for additional information.

5 New Hire Documentation required before employment
I-9 Employment Eligibility Verification Form. This form and THE required documents must be provided prior to hire date. Drug Screen (Safety Sensitive Employees Only) (This will be paid by the employee and can use the Physician of choice) Physical Form (Safety Sensitive and Full Time Employees Only)

9 Employee Benefit Forms Required
Blue Cross/Blue Shield of AL Enrollment - FULL TIME ONLY The cost for health and dental insurance is provided by St. Clair County. Booklets regarding coverage are available on the County’s website. ERS Enrollment Member Information form – FULL / PART TIME ONLY Mutual of Omaha Enrollment Form – FULL TIME ONLY The county provides a Short Term Disability Policy for the employee. The county also provides a Basic Life and AD&D policy for the employee, employee spouse, and children. Additional coverages are available for purchase at the employees expense. VSP VISION FORM – FULL TIME ONLY Vision coverage is available for purchase by the employee. Family coverage is $17.09 per month and single coverage is $8.95 per month.

There are additional insurance policies and benefits available for purchase by the employee. These can be viewed on the website under employee tools.
