Blue Cross/Blue Shield of AL Enrollment - FULL TIME ONLY
The cost for health and dental insurance is provided by St. Clair County. Booklets regarding coverage are available on the County’s website.
ERS Enrollment Member Information form – FULL / PART TIME ONLY
Mutual of Omaha Enrollment Form – FULL TIME ONLY
The county provides a Short Term Disability Policy for the employee. The county also provides a Basic Life and AD&D policy for the employee, employee spouse, and children. Additional coverages are available for purchase at the employees expense.
VSP VISION FORM – FULL TIME ONLY
Vision coverage is available for purchase by the employee. Family coverage is $17.09 per month and single coverage is $8.95 per month.
